Independent Living Navigator (Social Worker): Givens Highland Farms

Givens Highland Farms, a leading nonprofit life plan community in beautiful Black Mountain, NC, is seeking a full-time salaried Independent Living Navigator (Social Worker). Reporting directly to the Director of Resident Services, this role will help support our growing independent living population. The schedule is Monday through Friday during regular business hours, with some nights and weekends on-call as necessary.

What you'll do:

  • Implement and evaluate planned social services activities in both group and one-on-one settings  
  • Provide referral and community resources to residents and their families  
  • Advocate for and uphold Residents' Rights  
  • Monitor the needs of residents on a regular basis and work with the Navigation Team to ensure they are in the appropriate level of care  
  • Develop supportive and caring relationships with residents 
  • Adhere to and uphold HIPAA regulations 

What you'll need:

  • College degree in Social Services or a related field 
  • A minimum of 2-4 years of experience working with older adults 
  • Experience using Microsoft Office, Outlook, and other computer programs
  • Excellent interpersonal and communication skills, time management skills, organizational skills 
  • The ability to work independently, proactively, and with teams as assigned 
  • The ability to work in a fast-paced environment  
  • Must hold a valid NC Driver's License

Compensation: $45,000 -$55,000 annually and includes our comprehensive benefits package!
