An immediate opportunity is available for a full time LPN with a work schedule mainly Monday through Friday hours. The Restorative LPN will work with the MDS RN to maintain and evaluate residents for restorative programs. The Restorative LPN will be responsible for quarterly data gathering of resident mobility/balance or contractures, assistive devices, safety assessments, equipment, lift assessments, and completion of section GG in the MDS. The Restorative LPN will be responsible for oversight of the restorative CNA team, and assist with training and guidance for any restorative program. This position will work with the Admission Coordinator to assist with safe transitions of care for residents in the Givens Health Center. The Restorative LPN will gather information pre-admission and follow residents through the process of discharge planning as needed. This position, with the oversight of an RN, is accountable for the delivery of patient care based on the nursing process of assessing, planning, implementing, and evaluating while ensuring a safe and therapeutic environment, and assuring that the highest degree of quality care can be provided to residents at all times.
Qualified candidates must possess an unencumbered Practical Nurse license for the state of North Carolina and possess a minimum of one year experience working with the geriatric population. Preferred candidates will have supervision experience as an LPN, preferably in geriatric nursing setting. The ability to work with minimal direct supervision is a must.
